Same as Tom, but I always add in a sentence about what I've enjoyed about working there (if anything), and how I'll miss working there (if it's true). You never know when you might want to go back there/work with the boss again/ask for a reference, so I try to leave on a positive note and soften the blow a little bit. If you hated it, you can put some waffle about it being a learning experience (learning you never wanna go back, haha).
To be honest, you don't need to say a lot. As Tom says, just say that you are writing to formally hand in your notice, and that your last day of service will be x date. You'll need to look at your contract to work out what your notice period is. If you plan to work your full notice and not try negotiate an early release, then you can put the date of your last day in the letter.
There is no requirement for you to give reasons. (Being new to your blog I take it that it is your choice to leave rather than feeling you have to?).
Some managers will want to try and talk to you about it, but some don't. So don't be upset if your manager just takes it and doesn't say much. Where I work, most of them just take it and pass it on to HR who then respond. (I work in HR btw)

Just tell it straight, be brief, and tell them what your last day is (one month from now or whatever). That's all you have to do. Then your manager will ask you for a chat and try and persuade you to stay. You say no, agree the leaving date and that's it.
